Enter the date of surgery performed for each surgical procedure. There are three date fields available to be used in conjunction with each definitive procedure performed.
Procedures for this date field include:
Surgery of the Primary Site
Scope of Regional Lymph Node Surgery
Surgery of Other Regional/Distant Sites
Note: These must be entered in chronological order.
