V.6.3.1 Pathologic T

Coding Instructions:

• Refer to the most current AJCC Cancer Staging Manual for coding instructions.

• Code the pathologic T as documented by the managing provider or first treating provider in the medical record.

• If the managing provider has not recorded the pathologic T, registrars will code the pathologic T based on the best information available.

• Code pTX is used for occult carcinoma of the lung.

• Use code 88 for pathologic T, N, and M as well as pathologic stage group when:

• Site/histology combination is not defined in the AJCC Manual.

• For in situ tumors that are not staged according to the AJCC Manual.
